You really, really hate weaving in the ends. Therefore, you should do it as you go along, because it's an occasional minute of annoying rather than thirty minutes of boring frustration.
At the moment, I'm working an afghan done in motifs, so it's not too frustrating to weave in the ends after finishing the motif. If I fail to do that, then when I do the assembly, there are So Many Ends. So many, many ends.
I dunno about the thumbs. The thing that seems to suggest it's not the crocheting is that the use of the hands is asymetrical. Left hand holds the tension and the work, right hand manipulates the hook. Both hands use the thumbs, but in different ways. But the pain and clicking is symmetrical. It doesn't rule it out, but it seems to make it less likely. On the other hand, it is the big change in the last month and a half.
